My internal was messed up during a thunderstorm i was using internal one first then i bought a usb cable one i just removed the internal one.
Here is the usb ones name - SmartUSB56 Voice Modem
And about the Color problem all of the things in your quote are fine. (I mean none of them are the problem)

Did you ever load drivers for this modem or did XP find it's own?
Was this modem working OK before the current problems?
Does attempting to open dxdiag continue to cause a shutdown?
& just out of curiosity did the monitor problem coincide with the thunderstorm?

That last crash was caused by the modem/drivers.

It would be interesting to compare other logs.
The only other idea I have at the moment is the graphics controller.
You might try to replace that driver:

I replaced the driver and it told me i needed to reinstall windows xp so i system restored i still can not fix the problem

You replaced which driver? Intel?
How/ what told you to reinstall XP?

Can you recall the exact error message?
Have you had more crashes? More dump logs? Any other problems?
