I work on Windows XP with the Outlook Express 6 for a long time without special problems. I use to empty the Deleted Items folder every day before closing my computer and it says always that the items will be “permanently” deleted. After emptying, the folder remains empty.
However about once a month, when looking in the Deleted Items folder I find there a long list of deleted items from about 2-3 weeks.
It seems that “permanent” has an unusual meaning for the OE6.
What happens and what should be done to avoid this

What type of email account? If IMAP, that might account for it. Also, are you sure those items were deleted 2 - 3 weeks ago? And not just 2 - 3 week old emails just deleted? It seems if you look every day and note the bin is empty, these deleted 3 weeks ago should not suddenly re-appear. Maybe, for a couple weeks, you should start looking in the morning before you start your day to make sure nothing is in there. But again, if IMAP accounts, you can delete them from your computer but they are not necessarily deleted from the email server.
